WACH                         PRESIDENT / PRINCIPAL EXECUTIVE OFFICER   DATE:             September 15, 2009   SUBJECT:     FLU SHOT CLINICS ____________________________________________________________________________________   Please be advised that the Board of Trustees of the New York State Teamsters Council Health and Hospital Fund, in conjunction with Joint Council # 46 , will provide “free” flu shots  to all eligible participants and their dependents (9 years of age and older).   This year’s flu shot clinic in our area is:    Joint Council # 46 Office (Teamsters Local 264 )     35 Tyrol Drive, Cheektowaga, NY 14227   on   Date: Sunday, November 15, 2009 Hours: 9:00am until 1:00pm       You must bring your medical ID card (Excellus Blue Cross/Blue Shield Benefit Card)  with you to verify eligibility for yourself and/or family members that are covered under your medical insurance.   Be advised that this is the “regular” flu vaccine, not the Swine Flu (H1N1) Vaccine. At this point in time, the New York State Teamsters Council Health and Hospital Fund have no plans to offer the Swine Flu Vaccinations.   We encourage everyone to take advantage of this opportunity to protect themselves this flu season.
